Steerable catheter repositioning of a pacemaker lead loop to enable tricuspid edge-to-edge repair. Εδώ το πρόβλημα δεν ήταν απλά “lead-related TR”. Ήταν mechanical prolapse του loop του LV καλωδίου δια της τριγλώχινας, που οδηγούσε σε σοβαρή TR.
Η στρατηγική μας:
- Reposition του loop με steerable catheter
- T-TEER (TriClip) για αποκατάσταση coaptation
- Και το πιο σημαντικό: το ίδιο το clip λειτούργησε ως “barrier”, αποτρέποντας το loop από το να κάνει prolapse στη βαλβίδα
Δηλαδή:
- From dynamic lead-induced TR → to stable valve geometry after TEER
Result:
- TR ↓ σημαντικά
- NYHA IV → II
- No lead issues
- Take-home: Fix the mechanism, not just the regurgitation.
- Proud of the team!
